Can you give me the contact info for an employee?
Our privacy policy prevents us from responding to requests for information relating to RBMC employees' names, phone numbers or e-mail addresses. However, if you send the request through "Contact Us", we will make every effort to forward your message to the employee.

Can I get my medical records?
Yes. However, we cannot handle the transfer of sensitive medical information online. In compliance with NJ Department of Health Regulations, we are required to keep records for 10 years after the last date of service. There are two ways to access your medical information. You may have your personal physician make a request to the medical center, or you can request the record by providing the following information:
- Social security #
- Date of Birth
- Last date of service
- Complete name at time of service
- Signature with current date
- Brief statement of information requested
- Fax to the medical records department: (732)324-3160
The cost for obtaining a copy of a medical record is $1.00 per page with a $10.00 service charge. However, your doctor can request that we send the record to him or her at no charge. You must authorize your doctor to do this.

Can you send me a copy of my birth certificate?
No. Birth certificates are handled by the Perth Amboy Department of Health. They can be contacted at:

Extended birth certificates with time, weight and length may be available from NJ Dept of Health.

I attended the CEG School of Nursing and would like to know how to obtain an official copy of my transcripts.

I would like to make an appointment to donate blood. How can I do that?
