Patient Responsibilities
Statement of Patient’s, Parent’s, Health Care Representative’s, and/or Guardian’s Responsibility
In order for us to provide you, the patient, with the optimal quality of care, we ask that you comply with the following responsibilities:
1. You, or your family, will provide accurate and complete information about your present complaints, past illnesses, hospitalizations, medical devices, medications and other matters relating to your health history.
2. You will cooperate and follow the care prescribed or recommended by your physician, nurses, or other health care professionals.
3. You will notify your physician or nurse if you do not understand your diagnosis, treatment or prognosis.
4. You will advise your physician, nurse or other health care professional of any dissatisfaction you may have regarding your care at the hospital.
5. You will provide complete and accurate information necessary to process a claim with your health insurance carrier. You will assume financial responsibility for services rendered, either through third party payers (your insurance company) or through self-payment for services not covered by your insurance company.
6. Your will not take drugs which have not been prescribed by your attending physician and administered by hospital staff; and you will not complicate or endanger your health status by consuming alcoholic beverages or toxic substances during your hospital stay.
7. You will abide by hospital rules and regulations and be considerate of the rights of other patients and hospital personnel. The medical center is a smoke-free environment.
8. You will respect medical center property as well as the private property of other patients and hospital personnel. Hospital property is not to be removed from the premises.
9. You will be courteous to your physician and medical center staff.
