You Have the Right to the Best Care
Here at Raritan Bay Medical Center we understand your medical information is private and confidential. Further we are required by law to maintain the privacy of any individually identifiable information that we receive from you or others that relates to your past, present and future physical or mental health, healthcare you receive, or payment for your healthcare (your “Protected Health Information”). At the time of your admission to the hospital, you received a brochure, “Raritan Bay Medical Center Notice of Privacy Practices,” outlining our practices as required under the Health Insurance Portability and Accountability Act (HIPAA). All hospital employees and members of the medical staff are committed to complying with our privacy policies.
Among other things, you have the right to:
- Request restrictions on our uses and disclosures of your Protected Health Information for treatment, payment and healthcare operations.
- Reasonably request to receive communications by alternative means or at alternative locations.
- Inspect and copy certain Protected Health Information contained in your medical and billing records or in any other RBMC records used to make decisions about you.
- Request an amendment to your Protected Health Information. Please know we may deny your request for amendment, in certain circumstances.
MYRBMC Patient Portal
The MYRBMC patient portal is a free and secure online website, available at myrbmc.rbmc.org, providing patients convenient 24-hour access to personal health information from any location with an Internet connection through a smartphone, tablet or PC. Accessing the portal through a username and password, patients can view information such as laboratory and radiology results and chart these results over time. Patients also can share their personal health information with their family physician, consulting physicians and other providers involved in their care. On the portal, patients can read up on diseases and conditions, tests and surgeries, injuries and first aid, treatment options, exercise and nutrition, and medications. With high-resolution videos, downloadable flyers, and physician authored articles on just about every healthcare topic, these SmartEngage™ features help consumers take the guesswork out of good health.
Health Information Exchange
Consistent with Federal Regulation, RBMC has partnered with Jersey Health Connect, which is a Health Information Exchange (HIE). An HIE is governed by a strict set of rules designed to protect patient confidentiality, privacy and security. The purpose of an HIE is to allow physicians and healthcare facilities to share your clinical information electronically. The goals of this exchange are to reduce medical errors and eliminate redundant care. Ultimately, the HIE also allows you to access your medical data and become a more active, informed participant in your overall care. You are entitled to opt-out of this electronic health exchange by contacting Jersey Health Connect at 855-624-6542 or at www.jerseyhealthconnect.org.

Statement of Patient’s, Parent’s, Healthcare Representative’s, and/or Guardian’s Responsibility
In order for us to provide you, the patient, with the optimal quality of care, we ask that you comply with the following responsibilities:
- You, or your family, will provide accurate and complete information about your present complaints, past illnesses, hospitalizations, medical devices, medications and other matters relating to your health history.
- You will cooperate and follow the care prescribed or recommended by your physician, nurses or other healthcare professionals.
- You will notify your physician or nurse if you do not understand your diagnosis, treatment or prognosis.
- You will advise your physician, nurse or other healthcare professional of any dissatisfaction you may have regarding your care at the hospital.
- You will provide complete and accurate information necessary to process claim with your health insurance carrier. You will assume financial responsibility for services rendered, either through third-party payers (your insurance company) or through self-payment for services not covered by your insurance company.
- You will not take drugs that have not been prescribed by your attending physician and administered by hospital staff; and you will not complicate or endanger your health status by consuming alcoholic beverages or toxic substances during your hospital stay.
- You and your family will abide by hospital rules and regulations in place to support quality care for patients and a safe environment for all individuals in the hospital, and be considerate of the rights of other patients and hospital personnel. The medical center is a smoke-free environment.
- You will respect medical center property, as well as the private property of other patients and hospital personnel. Hospital property is not to be removed from the premises.
- You will be courteous to your physician and medical center staff.
- To treatment and medical services without discrimination based on sex/gender, race, age, religion, ethnicity, disability, creed, color, national origin, marital status, nationality, atypical hereditary cellular or blood trait, sexual orientation, gender identity or expression, veteran status, diagnosis, in sum or substance ‘any other category protected by state or federal law’, ability to pay, or source of payment.
